Re-Enroll for Next Year!

 

We, the faculty and staff of Admiral Farragut Academy, would like to extend our appreciation for your trust in us as a community in educating your child. As a school we continue to strive for a low teacher-to-student ratio, state of the art technology, and to implement current trends in curriculum.

Step One: Contract & Payment

Download and sign the Re-Enrollment Contract
The contract must be signed by both parents/guardians, if applicable.

Pay the tuition deposit and submit the contract to the Business Office.
We accept cash, check or wire transfers.
Elementary School Tuition and Fees
Middle and High School Tuition and Fees
 

If a Payment Plan was chosen, complete the TMS Payment Plan Form.
Available for domestic families only. Not available for international families.

Return all of the forms above and payment to the Business Office as soon as possible to reserve your space at Admiral Farragut Academy. 

Step Two: Additional Forms & Documents

Everything you need to submit for the Re-Enrollment Process is located in the drop-downs below. If you have multiple students in different divisions the required forms or documents may differ.

Fax, mail or hand deliver the forms below to the division office by July 1.

Student in the Elementary School

Step One: Contract & Payment

Pay the tuition deposit and submit the contract to the Business Office.

If a Payment Plan was chosen, complete the TMS Payment Plan Form.

Step Two: Documents & Forms

Fax, mail or hand deliver the following forms to the Elementary School Office.

  • Health Insurance Card (copy of the front and back)

DAY Student in the Middle School

Step One: Contract & Payment

Download and sign the Re-Enrollment Contract
The contract must be signed by both parents/guardians, if applicable.

Pay the tuition deposit and submit the contract to the Business Office.
We accept cash, check or wire transfers.

If a Payment Plan was chosen, complete the TMS Payment Plan Form.
Available for domestic families only. Not available for international families.

Return all of the forms above and payment to the Business Office. 

Step Two: Documents & Forms

Fax, mail or hand deliver the following forms to the Middle School Office.

  • Updated Immunization Form (copy of front and back, if applicable)
    ** Out of State or International Students - Florida law requires that all students be in compliance with Florida Immunization Guidelines to attend school in the state of Florida. Within 30 days of the first day of school, all out of state and international students must have the appropriate vaccines with the exception of those with a  religious or medical exemption. If the parent is unavailable to take their child to the Florida Health Department, transportation will be provided by an AFA employee who will stay with the student and then bring them back to school.
     
  • Health Insurance Card (copy of the front and back)
    ** International Students - Proof of medical insurance through a United States based insurance carrier is required. The school can provide coverage at a cost of approximately $800.00 (subject to change) for the school year. Coverage begins once the student begins classes and ends the last day of the school year.
